What is Case Management?

Case management involves connecting an individual to the services they need. Social workers or case managers have extensive knowledge of the resources available to the community. They serve vulnerable populations by providing them with information, assisting them in filling out paperwork or applications, & otherwise helping them navigate the social service system.

Why Case Management?

Services vary based on the organization, but most places offering case management assist with:
